Application for Make-up Exam
The makeup examination application is exclusively applicable to currently enrolled MIS students who, for a valid reason, were unable to take the scheduled exam(s). Please visit the Academic Calendar for deadlines. Make sure to contact first your FIC before filing your Application for a Make-up Exam.
Application Requirements:
1. Completed Application Form for Makeup Examination.
Procedure to Apply for Makeup Examination:
Step 1: Submit/Email the duly filled application form for the makeup examination to the Faculty Secretary (fics-secretary@upou.edu.ph). Ensure to include the MIS Program Support (misprogramsupport@upou.edu.ph) as CC. Additionally, provide a copy to the Records Section (records@upou.edu.ph).
Step 2: Await an email from the Faculty of Study containing further instructions.
